Restores Deep Staining from Amalgam Tattooing

Deep staining from amalgam tattooing presents one of the most difficult restoration situations to clinicians. In this case only the Transcend composite Universal Body shade was used to replace the amalgam, no blocker needed. Note the excellent color blending of the preserved oblique ridge.

No staining or layering was needed

Patient presented with discolored and defective distal buccal tooth color restoration on tooth #11 and mesial buccal on #12. Secondary caries beneath #12. Gingival recession caused the black triangle space seen.

Defective restoration and caries were removed. Darker-yellowish dentin can be seen at the cervical area. Translucent enamel present.

The preparation was etched and bonded per manufacturers’ instructions, then Transcend composite Universal Body shade was placed to reduce interdental space. Transcend composite is able to mimic surrounding tooth color. No staining or layering was needed.

Both teeth were restored with the same single shade composite

20-year-old failed composite restorations on teeth #19 and #18. Note the microleakage on the margins between the restoration and the teeth in both cases. Proximal boxes on tooth #19 were considerably deeper than the Class I preparation on tooth #18.

Patient presented with occlusal caries/defective restoration on tooth #13 and occlusal caries on #14. Caries and defective restoration removed and preparations filled with Transcend composite. Composite was adjusted and polished up beautifully

Patient presented with a failing composite restoration. Filling was removed, which revealed tattooing and a mesial crack. The mesial crack was still a small, manageable size and therefore a direct bonded restoration could be done instead of an indirect crown restoration. Products used: Ultra-Etch etchant, Consepsis solution, and Peak Universal Bond adhesive. A very thin layer of A3 PermaFlo flowable composite was used for the initial layer. Transcend composite Universal Body shade was used to restore the tooth. Composite was adjusted according to occlusion and polished up beautifully.

When we created Resin Particle Match™ technology, we worked to make the refractive indices of the resin and the filler particles similar to each other so Transcend composite Universal Body shade is able to take on the shade of surrounding dentition. The refractive index is a ratio that describes the speed of light through a material. This ratio determines how we perceive color. When the refractive index is tuned to the filler particles, the composite is better able to reflect the shade of the tooth around it.

Not only does the Resin Particle Match technology allow for a better chameleon effect, but thanks to careful opacity tuning, Transcend composite provides better overall esthetics and, unlike most of the competition, you don’t need a blocker shade. Transcend's unique opacity tuning helps it look better in larger restorations.
